TO SEARCH BY NAME:

1.  Click on the button with the magnifying glass. Then click LASTNAME.
2.  Enter the last name you are searching for. 
3.  Click the "GO" button.

To further refine your search, include FIRSTNAME as a second search choice. You may also search by any of the other fields listed when you click the magnifying glass button.
